Kids Zone
Region 13 follows AYSO’s Kids Zone — ten standards for behavior at games and events. See /families/pledge/.
Concussion Information Sheet
Concussion risks and removal protocols. Required by California law.
Sudden Cardiac Arrest Information Sheet
SCA risks and warning signs. Required by California law.

How to Submit
Sign the forms and upload through the digital form submission portal.
Reimbursement Requests
Volunteers can request reimbursement for approved expenses using the reimbursement form.
Withdrawal Requests
To withdraw from a program and request a refund (if eligible), complete the withdrawal form.
